CONSENT AND PERMISSION  

I, ________________________________________, hereby consent to CAST, Inc., permission to use and reproduce video footage (including still frames excerpted there from) taken of (the “Footage”) and certain photographs taken of (“Photographs”) (the “Footage” and “Photographs” which may be collectively referred to herein as the “Materials.” I grant permission to CAST, Inc. to share said “Materials” on its websites or in other venues (such as published materials, publisher websites, YouTube etc.) my name, likeness, image, voice, my presentation and the performance of the presentation.

I grant CAST a royalty free, perpetual license to such material and understand that I will receive no compensation for my performance or my Materials. In doing so, I release CAST, and its assignees and designees, from any and all claims and demands arising out of or in connection with the use of these materials, including but not limited to any claims for copyright infringement, defamation, invasion of privacy, compensation, or right of publicity. I also recognize that CAST may not have control of the uses of the Materials that are distributed (e.g., pirated materials), and I agree not to hold CAST responsible for use and reproduction by others.
